CURRENT CLINICAL PRACTICE OF PHYSIOTHERAPY FOR MANAGEMENT OF PATIENTS WITH SHOULDER IMPINGEMENT SYNDROME IN SAUDI ARABIA: CROSS SECTIONAL STUDY

Abstract : Introduction: Shoulder Subacromial Impingement Syndrome (SSIS) is one of the most frequent causes of shoulder pain. Evidence supported the use of conservative treatment in the management of patients with SSIS, especially physical therapy interventions. Clinical practice guidelines indicated that physical therapy interventions, including therapeutic exercises, manual therapy, patient education and advice were recommended as the most important interventions for the management of SSIS. However, to date, no study has investigated the current clinical practice of physical therapy in Saudi Arabia for the management of patients with SSIS. Methods: Physical therapists in Saudi Arabia were invited to participate in an online survey via Saudi Physical Therapy Association between May to December 2021.The developed online survey consists of 36 questions divided into five sections: eligibility, demographics, clinical practice regarding SSIS, barriers and facilitators of clinical practice. Descriptive and logistic regression analysis test were utilized to assess the association between participants’ demographics and the use of the evidence-based guidelines for management of SSIS. Results: A total of 313 completed the survey. Results revealed that 95% of physical therapists were aware of the most recent evidence-based practice guidelines for the management of patients with SSIS. 99% of the participants prescribed therapeutic exercises, 98% educated and advised their patients, 81% applied manual therapy techniques, and 67% applied electrotherapy modalities. Univariate logistic regression was used to determine the association between demographic data and clinical practice of the participants and showed that no significant relationship variables except the qualification of the participants. Conclusion: Overall, physical therapists in Saudi Arabia mainly used therapeutic exercises and advice in managing patients with SSIS, reflecting the awareness of the current evidence-based guidelines for managing patients with SSIS. However, participants were not fully following the guidelines of management of patients with SSIS. Furthermore, future studies are recommended with a larger sample size. Also, audit visits to sustain adherence to the clinical practice guidelines.
